WAIVER AND LIABILITY RELEASE

By registering a child for any program, class, camp, event, or activity offered by the Bahamas Youth Table Tennis Academy (“the Academy”), the parent or legal guardian (“Parent/Guardian”) agrees to the following terms:

1. Assumption of Risk
I understand that participation in table tennis training, physical conditioning, and related activities involves inherent risks, including but not limited to:

Slips, falls, or collisions

Muscle strains or injuries

Accidental contact with equipment or other participants

I voluntarily assume all risks associated with my child’s participation.

2. Release of Liability
To the fullest extent permitted by Bahamian law, I release and hold harmless the Academy, its coaches, staff, volunteers, and representatives from any liability, claims, demands, or actions arising from:

Injury

Illness

Accidents

Property loss

This release applies whether the incident occurs during training, events, travel, or any Academy‑related activity.

3. Medical Authorization
I authorize the Academy to:

Provide basic first aid

Seek emergency medical care if necessary

Contact emergency services and my listed emergency contact

I understand that I am responsible for any medical expenses incurred.

4. Parent/Guardian Responsibilities
I agree to:

Ensure my child follows all safety rules

Inform the Academy of any medical conditions, allergies, or limitations

Pick up and drop off my child on time

​

5. No Guarantees
I understand that participation does not guarantee skill improvement, team placement, or competitive results.

​

6. Agreement
By enrolling my child, I acknowledge that I have read, understood, and agree to this Waiver & Liability Release.
